2023 Priorities for Monitoring COVID-19 in Marin

As the pandemic evolves, so have the tools to track the virus and its impacts. In 2023, Marin County Public Health will be focusing pandemic surveillance on five key elements, which will continue to be displayed on the Coronavirus Surveillance Web Page. Virus levels in wastewater, which covers nearly 90% of the Marin population, will be used to describe community transmission rates. To assess COVID-19 severity and healthcare burden we will track hospitalizations and deaths. For planning and forecasting, we look for changes in locally circulating variants, and follow national and global trends closely. We will continue to actively monitor for outbreaks in congregate settings, including schools and long-term care facilities, where early detection and intervention can prevent transmission.

Federal and Local COVID State of Emergency

On January 11, 2023, the U.S. Department of Health and Human Services extended the national COVID-19 state of emergency, which had been set to end in late January. In place since January 2020, the declaration has been extended 12 times. The latest extension is in response to rising daily cases nationally and the emergence of the XBB 1.5 variant. Marin’s and California’s states of emergency are both slated to end on February 23. As of this time, there are no plans to extend locally beyond this date.

Booster Effectiveness: Global Data Validates Marin Findings

Marin County Public Health has been sharing local data about the effectiveness of the bivalent booster in protecting Marin residents. Now, larger global studies are validating this conclusion, including a new Israeli study published in Lancet. Among more than 700,000 participants age 65 and older, who either had the bivalent or did not, there was an 81% reduction in COVID hospitalizations and an 86% reduction of deaths. During over 70-days follow-up, there were 73 deaths in those without the bivalent booster and 1 death of a participant who received it. The bivalent booster is also effective against XBB 1.5, which is likely to become the next dominant strain locally. In Marin, 62% of residents age 65 and older have received the bivalent booster.

Know your COVID-19 Community Level

The CDC’s COVID-19 Community Levels are a tool to help communities decide what prevention steps to take based on the latest data. Levels can be low, medium, or high and are determined by looking at hospital beds being used, hospital admissions, and the total number of new COVID-19 cases in an area. Take precautions to protect yourself and others from COVID-19 based on the COVID-19 Community Level in Marin or other community in which you live, work or visit.

If you are at high risk of getting very sick, wear a high-quality mask or respirator (e.g., N95) when indoors in public. If you have household or social contact with someone at high risk for getting very sick, consider self-testing to detect infection before contact, and consider wearing a high-quality mask when indoors with them. Maintain ventilation improvements. Avoid contact with people who have suspected or confirmed COVID-19. Follow recommendations for isolation if you have suspected or confirmed COVID-19. Follow the recommendations for what to do if you are exposed to someone with COVID-19.

People may choose to mask at any time. People with symptoms, a positive test, or exposure to someone with COVID-19 should wear a mask.
