Replacement of Lost Vaccination Card
Marin County Public Health (MCPH) is strongly encouraging residents to take advantage of the Digital Vaccine Record available from the State of California. You can use the digital record as proof of vaccination wherever you go.
Marin County Public Health (MCPH) will provide replacement CDC COVID-19 vaccination cards or will make corrections to the COVID-19 immunization record for individuals who received their primary vaccinations from MCPH. Submit your request for a replacement CDC COVID-19 vaccination card or COVID-19 vaccination correction using the Vaccine Card Request Form.
If you did not receive your primary vaccinations from MCPH, please go to the provider of your COVID-19 Vaccination Primary Series to obtain a replacement CDC COVID-19 vaccination card or to have a COVID-19 immunization record correction.
Speaking of Digital COVID-19 Vaccine Records...
Did you know you can easily download a digital version of your COVID-19 vaccine card onto your cellphone? The digital record includes the same information as the paper vaccine card. The QR code confirms the vaccine record is officially issued by the State of California. 
- name,
- date of birth,
- mobile number or email,
- create a 4-digit pin.
You will then receive a text with a link. Select the link and re-enter the 4-digit pin you chose. This final step will take you to your scannable QR Code and details that can be confirmed visually without a QR reader. Save the record to your camera roll or in an app like wallet.
Done! Present the record wherever proof of vaccination is requested.
Know your COVID-19 Community Level
The CDC’s COVID-19 Community Levels are a tool to help communities decide what prevention steps to take based on the latest data. Levels can be low, medium, or high and are determined by looking at hospital beds being used, hospital admissions, and the total number of new COVID-19 cases in an area. Take precautions to protect yourself and others from COVID-19 based on the COVID-19 Community Level in Marin or other community in which you live, work or visit.
